How Should I Take My Health Risk Assessment?

The Health Risk Assessment (HRA) is one of the first activities you must complete after signing up for the wellness program. The outcomes of the HRA give an insight into the risk levels and possible risk factors. 


The Health Risk Assessment (HRA) can be taken from the web platform or the MyHealthPlus app.


From the web platform:

1. Log in to the Wellness360 web platform using the correct credentials.


2. Click on ‘Complete the HRA’ from the Onboarding Checklist on the portal dashboard


or

Navigate to the Health Section on the side menu bar and click on ‘Health Assessment’.


3. You will be redirected to the Health Assessment page.


4. Click on ‘New Assessment’ at the top right to start taking a new health assessment.


5. In case, you have partially completed your health assessment, complete it here.


6. You can also edit your answers from the assessment reports displayed for each assessment parameter.


 7. Complete the full questionnaire in all the assessment categories with accurate answers to generate a Wellness Score and to identify the risk levels.




From the MyHealthPlus app:

1. Install the MyHealthPlus app and log in with the correct credentials.


2. Tap on ‘Health’ from the bottom toolbar.


3. On the 'Health' page, tap on the ‘+’ icon to start taking a new assessment.




4. If you have partially completed your health assessment, complete it here.


5. Once the assessment is completed successfully, you will receive a Wellness Score and average risk level, displayed above the Health map.

6. Tapping on any parameter like Nutrition, Exercise, etc., on the map will redirect you to a page with the risk level results of the specific parameter and the recommended suggestions.
