How to Upload Proof for Activities from the Web Portal?

A few wellness activities of the program may require employees to submit 'Proof of Completion' to mark the activity as complete. Most of the activities could be preventive screenings or wellness events. The activities that need proof to be uploaded have 'Yes' under the specific column across each activity in the Activities box on the dashboard.



Uploading Proof on a Computer

1. Click a high-resolution picture of the document proof of the wellness activity or reference number, and email it to yourself. Download the picture from your email to your computer.


2. Navigate to the Activities section in the Dashboard of the wellness platform. The list of your wellness activities will be displayed. If your activity needs proof to be uploaded, it will be mentioned as 'Yes' across it in the 'Proof Required' column.




3. Click on your specific activity to be redirected to the specified Activity page. Click on Update Status at the top right.


4. Click on ‘Choose File’ and select the document from your computer. Click ‘Open’ and upload the document. The document needs to be in one of these formats: Word, PDF, JPG, or PNG.



5. After uploading the document, click ‘Submit Request’ to mark the activity as complete.


6. You will receive a confirmation message at the top right. The message indicates your document proof was submitted and the activity marked as complete.


7. It may take 7 - 10 working days for the uploaded proof to be processed.


Uploading proof for Activities using a Smartphone

The easiest method to upload activity proof on a smartphone is by using the browser on your smartphone (not the MyHealthPlus app!).


1. Click a high-resolution picture of the document proof of the wellness activity or reference number, and save it on your smartphone.


2. Open your wellness platform on your phone's browser, and log into your wellness account. 


3. Go to the activity and click to open. Click Update Status. Then Choose File.



4. Choose the image from your photo library. If it is any other file, upload it from the file manager. Once the file is uploaded, click on Submit.


5. You will receive a confirmation message. The message indicates your document proof was submitted and activity marked as complete.

                                                         


6. It may take 7 - 10 working days for the uploaded proof to be processed.
