How to Upload Proof for Activities in the Wellness360 Mobile app?

Certain wellness activities require employees to submit a “Proof of Completion” in order to mark the activity as complete. This article walks you through the simple steps to upload your proof directly from the Wellness360 mobile app.

Steps to Upload Proof

  1. Open the Wellness360 mobile app and sign in with your credentials.

  2. From the home screen, tap Health in the bottom-left section and select Wellness Activities.


  3. Now, Find and tap the activity for which you need to upload proof. Within the activity details, scroll down and tap Update Status.


  4. In the Add Activity Request pop-up, tap the Upload icon. Then, Use any of these two options to upload the Proof file:

    • Select File: Choose an existing document or image from your device.

    • Take Photo: Capture a new, high-resolution photo of your proof.


  5. If you’d like to include any notes or context, enter them in the Comments field.

  6. Tap Save to submit your proof. 




Viewing Your Request Status

After submission, you can track the progress of your proof request at the bottom of the activity page. Statuses include:

  • Pending for Review

  • Approved

  • Not Approved
