Tracking and reviewing physician forms submitted by users is important for maintaining accurate health records and ensuring compliance with organizational requirements. Wellness360 provides a streamlined process to access and review these forms efficiently.

Accessing Physician Forms via PHI Login

• Log in to the Admin Platform using your PHI login credentials.

• In the left section of the page, click on the Health Records icon.

• You will be redirected to a page divided into two sections: 'Biometrics' and 'Physician Form Submissions'.

• Click on Physician Form Submissions to view all entries.

Viewing and Approving a Specific Physician Form

• Locate the user entry for the physician form you wish to review.

• Under the Actions column, click on the Arrow icon.

• Click on the three horizontal dots icon, then click View Details.

• A pop-up will appear displaying a preview of the submitted form.

• The pop-up also provides options to manually enter data from the form.

• For a Pending Form, review the entered details and ensure there are no discrepancies.

• If the information is correct, toggle ‘On’ the Approve Submission option.

• Click on the Update icon to approve the physician form.

By following these steps, admins can efficiently review, verify, and approve physician form submissions, ensuring accuracy and compliance with health records management.
